Elevyo Healthfinder – Terms of Service
1. Acceptance of Terms
By creating an account, accessing, or using the Elevyo Healthfinder™ platform, you agree to these Terms of Service (“Terms”) and to our processing of personal data as described in our Privacy Policy. If you do not agree, do not use the platform.
2. About Elevyo Healthfinder
Elevyo Healthfinder™ is a digital health profiling tool designed to support healthcare providers (“HCPs”) and individuals in delivering proactive care based on positive lifestyle and behaviors and prevention. It does not provide medical diagnosis or treatment.
3. Eligibility
You must be at least 18 years old. HCP accounts must only be created by licensed professionals or authorized clinic staff. If you are acting on behalf of an organization, you confirm that you have the legal authority to do so.
4. Your Account
You are responsible for maintaining the confidentiality of your login credentials. You must notify us immediately if you suspect unauthorized access. You must ensure that all information provided is accurate and up to date.
5. License and Use Restrictions
We hereby grant you a non-exclusive, non-transferable license to use the Elevyo Healthfinder™ platform within the scope of your account type. Accounts are for individual named users only and may not be shared. You may not resell, sublicense, copy, modify, reverse-engineer, or distribute the platform without our prior written consent.
6. Permitted and Prohibited Use
Permitted:
- HCPs may use Healthfinder™ for assessments, prevention planning, and client engagement.
- End-users may use Healthfinder™ for personal health insights and to share relevant information with their HCP.
Prohibited:
- Uploading false, misleading, or unauthorized data.
- Using Healthfinder™ for unlawful, harmful, or discriminatory purposes.
- Attempting to disrupt or compromise platform security.
7. Payment and Contract Period
Unless otherwise agreed in a separate Service Agreement, individual subscriptions are billed monthly in arrears via automated debit/credit card payment. The contract period matches your chosen payment cycle and renews automatically unless terminated in writing at least 30 days before the end of the period.
8. System Requirements
Healthfinder™ is optimized for current versions of modern browsers and supported mobile operating systems. Use of outdated systems may result in reduced functionality.
9. Data Protection and Privacy
Our Privacy Policy explains how we process personal data. For organizational customers, a Data Processing Agreement (“DPA”) governs processing on behalf of the customer. By entering personal data of third parties (e.g., clients or patients), you confirm that you have the legal right and necessary consent to do so.
10. Medical Disclaimer
Healthfinder™ is not a substitute for medical advice, diagnosis, or treatment. Healthcare professionals remain solely responsible for clinical decisions, medical record-keeping, and compliance with local healthcare regulations.

12. Third-Party Services
We are not responsible for the content, availability, or policies of third-party websites or services linked within Healthfinder™. Your use of such services is at your own risk and subject to their terms.
13. Availability, Support and Error Reporting
We aim for high availability but do not guarantee uninterrupted service. Standard support is available by email on normal business days in Sweden between 09:00–15:00 CET. For enterprise customers, specific service levels are defined in a separate Service Level Agreement. Error reports can be sent to support@elevyohealth.com.
14. Ownership and Intellectual Property Rights
All content, software, and trademarks belong to Elevyo Health AB. You are only granted the rights expressly set out in these Terms and the Service Agreement entered with Elevyo Health AB.
15. Limitation of Liability
Subject to the terms of the Service Agreement, Elevyo Health AB is not liable for indirect, incidental, or consequential damages resulting from your use of Healthfinder™.
16. Termination
We may suspend or terminate your account if you breach these Terms, the terms of the Service Agreement or any applicable laws or regulations. Termination of these Terms or the Service Agreement does not affect any obligations to pay for fees already incurred.
17. Changes to these Terms
We may update these Terms periodically. Changes take effect upon notification through the platform or email.
18. Governing Law and Disputes
Any dispute, controversy or claim arising out of, or in connection with, these Terms, or the breach, termination or invalidity thereof, or any non-contractual obligations arising out of or in connection with these Terms, shall be finally settled by arbitration in accordance with Clause 14 of the Service Agreement.
